Regular checks - Weight / Blood Pressure / Waist Circumference

Bloods including 3 monthly HbA1c

Foot checks and referral to podiatry

Referral for diabetes specific eye checks

Regular urine tests for microalbumin

Commencement and titration of  diabetes medications including insulin

Referral to diabetes specialist (Endocrinologist) where required
